Stanford Member Services Specialists are health plan experts who are here to make your experience as stress-free as possible. If you have a question about the plan or need to find doctors, services, or locations, you can turn to a friendly and knowledgeable Member Services Specialist for guidance.

When you enroll in a LifeConnections plan, you can contact a Member Services Specialist at 1-844-845-8078 (Monday – Friday, 6:30 a.m. to 7 p.m. Pacific Time) to record your selection. It is recommended that you select a personal doctor within 90 days from when your membership becomes active on January 1, 2018

If I enroll in a LifeConnections plan, may I continue to see my same doctor?
Do all family members have to select the same personal doctor?
Do I need a referral to see a specialist or other provider?

The plan allows you to see any physician in the network without authorization. When seeking specialty services, please work through your personal doctor to ensure a seamless transition. You’ll also have help looking for locations near you and making appointments by contacting a Member Services Specialist at 1-844-845-8078  (Monday – Friday, 6:30 a.m. to 7 p.m. Pacific Time) for any questions.

What should I do for non-emergency care after hours or when my personal doctor is unavailable? Are there urgent care options?

You can call our Member Services Specialists at 1-844-845-8078 (Monday – Friday, 6:30 a.m. – 7:00 p.m. Pacific Time) to find other available providers when you’re unable to see your personal doctor (primary care physician). When you need immediate care for a non-emergency situation, we have urgent care centers throughout the Bay Area.

Are there other hospitals in the plan besides Stanford Health Care or Lucile Packard Children’s Hospital at Stanford?
