Yes, when you need immediate care for a non-emergency situation, Stanford Express Care San Jose is close to Intel and is open seven days a week from 9 a.m. to 9 p.m. You also have access to several urgent care centers throughout the Bay Area.
Yes, you have access to Stanford Health Care Tri-Valley in the Tri-Valley area, and other participating hospitals throughout the Bay Area. In any emergency, no matter where you are, you can go to the nearest emergency room for care.
Stanford Health Care Alliance Case Managers can help coordinate and manage your care. Our case managers are Registered Nurses with hands-on experience in hospital care as well as other areas of clinical practice. When needed, a case manager is part of your care team in developing a plan of care specifically tailored to your needs. A case manager will regularly review your case, make outreach calls, perform assessments and identify potential care gaps; then, when applicable, coordinate with multiple sources including providers, family, caregivers, vendors to address and close those gaps. With the goal of educating and preparing patients for self-management, case management is yet another way we provide personalized, high-touch care.

A Stanford Health Care Alliance Case Manager (a Registered Nurse) will reach out to you and perform an assessment, which is a series of specific questions related to your health. This assessment will aid the case manager in identifying your clinical needs and goals, in order to develop a plan of action to address them. Along with the care plan set by your providers and as part of our team-based approach to care, case management is an integral partner and touchpoint when multiple services are needed. Once mutual goals are achieved, the case can be closed.
